It's worth noting that there is a random factor applied to break points, making it possible that they will occur slightly before the table above, but in general chrisb is exactly right.

There was a fix for the lack of DLC PoIs, but this only affects new campaigns I think, so you'd have to have restarted during 1.2 to benefit from it. Well, I have to disagree here. The hit mechanic is not random. Yes, the hit is based on a random roll, but the actual numbers are not random at all. The game gives me all the information to see how the current hit chance is calculated, and I can then make decisions to either reduce those red number...

I think that's just because people haven't caught on yet. If you watch JoINrbs' most recent stream, he maxed out pistols on all of his A-team and it was pretty effective. Actually, with hidden perks, I'm pretty sure the way you 'solve' the decisions is "train good perks if available, otherwise train pistols". On average, the value of two pistol perks is going to be greater than the value of one bad offence/defence perk plus one random offence/defence perk....

Also if the region has been contacted it seems like vigilance is bound to a minimum of 2. I don't think this is actually true, but the resistance runs operations (which generates vigilance) before you contact a region, so most regions start with vigilance above 0. I think you're right that it has something to do with difficulty levels. Legendary is supposed to be hard.
