As you complete missions in the region, you will discover that faceless are stealing your supplies as the reason. (highlight to read) You will be able to counter this though the same mission that gives you the discovery.

From what I've seen, they become a faceless when you end your turn with in a blue move of them.

The mod you are looking for is Ammo as weapon mods. It doesn't appear to have any conflicts with LW. The mod puts all ammo into the weapon itself.
