I saw the same thing on a Supply Run mission yesterday. My issue was triggered by an enemy pod member catching fire while I was still concealed. It happened to me on Network Tower last night, but this was an auto evac situation, so I attributed it to that. The soldier lived as I completed the mission before his bleedout timer expired.

Somewhat related -- I'm seeing my modded weapons occasionally un-equip themselves and land back in the weapon pool. This seems to happen on Officers after training or learning new Officer perks, and perhaps also after AWC training. I haven't exactly verified how it's happening, but I've seen it now ...
