Under 1.2, I researched 3 facility leads and was shown the location of 3 facilities, one of which I destroyed.

Same campaign, now under 1.3, I have researched 2 more facility leads and have been given no facility locations.

Is it simply that there must only be 2 facilities on the map, researching 3 Advent upgrades, plus advancing the Avatar project?

you need to have contacted the region a facility is located in.
