The infiltration % and the days/hours indicated in the mission preparation screen do not match.
I.E.: if it says the mission still has 5 days until it expires and you put a squad with 4days 22hs, you won't get 100% infiltration.
How many hs less than the expiration do you need for 100% infiltration?

It varies due to flight time. The Skyranger takes a little time to drop off the squad and that time is a little unpredictable due to the random positioning of missions on the map relative to the avenger landing point in each region.
2-3 hours is often enough. Sounds like you found a particular mission location where 2 hours didn't quite cut it.
