Not sure if its an Xcom 2 bug or a Lw 2 bug. But in the most recent patch often civilians will spawn with white shirts covered in blood.
Now, I know its hard to clean up stains out of your shirts, but you'd think they'd have the courtesy of cleaning up their clothing before going out in public.
