Have also seen this with and without LW2.
In Evac missions I just made a habit of saving just before extracting the last soldier. On other mission I save once I feel the mission is close to its end.
I don't play ironman obviously.

You should expand Tip #4 a bit, in that it makes missions notably harder if you start them below 100% infiltration.
Especially in the early game, starting a mission at 20% because you want those 5 soldiers back ASAP might very well result in them not coming back at all...

Quick tip on sectoids:
You can break their mind control, psi zombies (and i guess panic) by flashbanging them (or otherwise disorienting them).
This really helps early on.

Changing the line CharacterBaseStats[eStat_UtilityItems]=3 both in [Soldier X2CharacterTemplate] and just below that for your difficulty should work. Keep in mind that the UI may not adjust in Squad loadout screen, so you might have to go into the soldier's specific loadout screen to access these sl...
